In partnership with Corporation for Supportive Housing (CSH), the League is hosting a training to address the following learning objectives.
1. Understand the position of housing as the leading social driver of health.
2. Build awareness of federal, state, and local housing programs, eligibility, and how to access them. This includes:
a. Emergency shelter,
b. Residential Assistance for Families in Transition (RAFT),
c. Federal and state subsidy vouchers,
d. Public housing and
e. Continuum of Care Resources,
3. Discuss levels of health and housing partnerships health centers can engage in to advance this work.
4. Understand the critical role of CHWs in addressing social drivers of health via community partnerships and upstream solutions.
